2019 entry form


Please read the following instructions carefully. The entry form itself is below the instructions.

Entries must be received by midnight on March 25th. All photos must be received by March 26th. All fees must be received by April 1st. Entries missing any of these deadlines will not be accepted. 

You fill out the entry form by answering the questions right on this page. Move between questions using the Tab key or your mouse. At the end there is a submit button. You can review and make changes to your answers up until you submit your entry. Make sure your email address is typed correctly. After you submit your entry, you will receive an email summarizing your answers and indicating any fees to be paid. Retain the email for your reference. Reply to the email to request changes, such as fixing typos, supplying new measurements, or withdrawing your entry. If you don't get the email, contact the Registrar.

You can prepare your answers before filling out the online form using the printable form. The Registrar does not want to receive paper forms. If you need help with the online form, ask questions via email or visit the Quilt Show table at the March meeting.

Your entry is incomplete without a digital colour photo of the quilted item. The entry form and photo may be submitted separately as long as the photo is identified with your name and the entry title. Your item does not have to be complete at the time you submit your entry, but the photo must be recognizable as to pattern, colour and style. If you choose to send your photo by email, the easiest way is to reply to the summary email you receive and attach the photo. If you send it in a separate email to the Registrar, please specify your name and the quilt title in the subject line The photo must be received by March 26th; otherwise, your entry will be withdrawn.  

There are two types of fees: entry fees for submissions in the main categories and written critique fees if you have chosen that option. Entries in the main categories are incomplete without the associated entry fee. You may pay fees by mail or in person at the quilt show table at one of the February through April meetings. Please ensure that your quilt is insured under your personal household policy. We will make every effort to protect your work, but we do not carry insurance to cover the value of the quilt.
